As a longevity medicine practice, our team reviews your history, sets personalized goals, and guides you with a proactive, evidence-based approach.

Medical insurance is billed for all appointments. Patients are responsible for their copay, coinsurance, and any applicable deductibles. These costs are in addition to the $250/month membership fee. Coverage varies based on your individual insurance plan.

Please note, Dr. Renata Gazzi’s monthly fee is $300.

Menopause Membership

If menopause is your primary focus, our Menopause Membership is the ideal entry point. Over three months, we take a deep dive into your symptoms, concerns, and goals — giving women in midlife the focused support they deserve.

From hormone therapy to lifestyle strategies, we create a personalized care plan tailored to your body, your needs, and your stage of life.

Medical insurance is billed for all appointments. Patients are responsible for their copay, coinsurance, and any applicable deductibles. These costs are in addition to the $250/month membership fee. Coverage varies based on your individual insurance plan.

Self-Pay Membership (no insurance)

Our self-pay membership is perfect for patients who do not wish to use their insurance, or who have an insurance plan we do not accept.

The $650 monthly fee with this membership option  includes visits with your provider and quarterly lab tests. Patients will be responsible for medication costs (may be able to use insurance) and other tests done outside of Innovative Vitality, as those costs are not part of the membership fee.
